We have a form that we have built that addresses safety concerns on-site. In that form there are 2 pages, one for reporting on the safety concerns, and one as a write-up to a subcontractor for violations. It would be helpful in a scenario where multiple subcontractors are involved with a concern, that they don't all see the write-up that was sent to just one of them. We want to be able to distribute the two forms independently.
