Currently forms distributed from Fieldview adopt a string of Metadata which has no meaning for the intended recipients. The form type title, project and reference are shown which is useful however it would be beneficial to have the form name or creation date showing on the distributed document file name.

We agree that this is a good idea. And you are also correct that this is a recurring request. I'll combine this with a similar naming convention so the votes will combine.
As an MVP we require the ability to have the form title included in the PDF file name. We have hundreds of check sheets that are sent to VfP when complete , yet we have to open each PDF to see what it is, and then rename it.
Ultimately - we would like tho be able to set this file name up to be BS1192 compliant - when integrating with VfP, if we have the BS1192 module switched on we would want the PDF to be named correctly so it could be uploaded.
This is similar to a request I have made a couple of times, though I would like to see the Form Location in the pdf file name
